This Health Coach role requires telephonic outreach to UHC members that have been pre-identified. Members enrolled in this program are primarily Adults, children and families, medically complex and require intensive medical and psychosocial support. Positions are responsible for establishing personal wellness plans focusing on members living healthier lifestyles. Roles will provide telephonic, online or in person support to member’s while assisting them establish wellness goals. Coordination of member care needs across the continuum begins with early identification of health risk factors, education, and prevention. Coordination also assures the provision of appropriate services in acute, home, chronic and alternative care settings, to meet the needs of members in the most cost-effective manner available.

Demonstrate an ability to deliver effective telephonic coaching in a manner that promotes behavior change for the Missouri Medicaid population
Be able to assist clients in creating a healthy vision and goals for their Health & Wellness program
Appropriately adapt telephonic coaching and education methods to accommodate the unique physical, psychosocial, cultural, age-specific needs of each member served
Demonstrate established customer service behaviors and standards
May require some field work- meeting members in homes, facilities if difficult to engage telephonically or at the request of management

4+ year degree with healthcare experience
2+ years of clinical/healthcare experience
2+ years of experience in direct education and / or coaching with individuals for disease prevention, health promotion and behavior change
Proficient in PC-based word processing and database documentation (Word, Excel, Internet, Outlook) and the ability to type and talk at the same time
Basic knowledge of disease prevention and basic understanding of exercise principles, nutrition and weight management
Have a dedicated work space from home
Full COVID-19 vaccination is an essential requirement of this role. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination prior to employment to ensure compliance
LPN or MA
Managed Care experience
Experience with government health programs (Medicaid / Medicare)
Ability to navigate multiple computer programs simultaneously
