The Population Health LPN (PH-LPN) is an expanded care management role that collaborates with patients, healthcare providers and patient's caregivers to ensure delivery of quality, efficient and cost-effective healthcare services for identified patients. The PH-LPN will assist the AHN patients in all aspects of care coordination and ensure the beneficiary is fully educated on all options and services with the goal of optimizing the patient's health status.
This position will be based out of American Health Network's administrative office in Noblesville, IN. The schedule is Monday - Friday, 8am-5pm.
Contributes to the assessment of the healthcare, educational and psychosocial needs of the patient and their family by gathering data.
Contributes to the development of the plan of care for the patient using a team approach by working collaboratively with the family, primary care provider, and other members of the healthcare team to ensure coordination of services.
Implements plan of care under the direction of RN and/or provider.
Identifies and coordinates referrals and other community resources by working with the primary care provider and other members of the healthcare team.
Implements system of care that closely monitors high risk patients to prevent/intervene early during acute exacerbations.
Ensures medication reconciliation is completed and documented for all assigned patients.
Performs follow-up calls following an assigned patient’s care encounter for issues, feedback and continued follow-up needs.
Assists with data collection and closing of care gaps of the quality metrics as assigned and assists the healthcare team in meeting all of the quality metrics.
Performs CPR/first aid on a patient and takes vital signs during an emergency situation.
Provides a stabilizing influence for the patient/family during treatment.
Facilitates problem-solving with the patient/family to mobilize patient resources.
Assesses the clinical condition of patient during all interactions.
Assesses the health status of the patient in conjunction with other members of the healthcare team for analysis and identification of health goals.
Assesses barriers to self-care (clinical, financial, social, etc.)
Identifies opportunities for process improvement in all aspects of patient care (under the direction of RN and/or provider care).
Advocates for patient/family in assisting the health care team in understanding patient’s rights.
Works cohesively with the healthcare team to link patients with the most appropriate resources.
Performs all functions and duties in compliance with state and federal laws, including but not limited to state licensure limitations, requirements and standards for an LPN.
Educates and reminds patient to utilize AHN’s Call Me First program by calling PCP office number after office hours, if non-emergent medical needs arise.
Ensures patient records are maintained in a confidential manner and in accordance with standard operating procedures.
Active, unstricted LPN license in the state of Indiana.
Minimum of 2 years of clinical experience
Previous experience in population health or managed care
Case management experience
Transition of care experience
Previous telephonic nursing experience
Experience with NextGen EMR software
