CVS Health Special Needs Unit Coordinator in Philadelphia, Pennsylvania

Job Description

The SNU Coordinator must have access to and periodically consult with the ABH of PA’s Medical Director and must work in close collaboration with the SNU and SNU staff.

Schedule is Monday-Friday. No weekends and no holidays!

Local travel within state of Pennsylvania 10-25%.

Fundamental Components

Responsible for reviewing state and/or health plan reports identifying members that are classified as “special needs” and ensures processes are in place to manage the special needs population . Is the primary point of contact for DHS for urgent special Needs member inquiries and facilitates and communicates that resolution to the State SNU Department.. Coordinates with all community based entities , CYS,OVR,ODP JPO, Initiates communication with the member’s Primary Care Provider, Department of Family Services Case Worker, School Nurse, and/or specialist provider as necessary to assess the clinical and/or social needs of the special needs member and refer to case management as appropriate. . Works with health plan management to develop and maintain the plan’s community resource manual. Completes State required reports for Special Needs population Participates in state directed meetings and/or in-services to provide education to the Special Needs Unit to ensure proper processes are used in the care and coordination of care for the “special needs” population. Performs other duties as required.

Background Experience

3 years experience in the Special Needs population required.

Ability to work independently.

Complies with all state requirements in the state where job duties are performed.

Strong verbal and written communication skills.

Proficiency with standard corporate software applications, including MS Word, Excel, Outlook and PowerPoint, as well as some special proprietary applications.

LICENSED ONE OF THE FOLLOWING with active state license in good standing in the state where job duties are performed:

Registered Nurse (RN)

Social Worker with LCSW

Licensed Professional Counselor

LMFT

EDUCATION

Bachelor's degree required.

Additional Job Information

The Special Needs Coordinator interacts with the Care Management team ,inclusive of the Special Needs Unit. Reports to the Director of Medical Management and works collaboratively with Plan CMO and all plan departments. as it relates to the Special Needs Population.
