The Case Management Coordinator provides concurrent review of patient care for the purpose of resource monitoring, allocation and coordination of continuing care. The coordinator supports the case management and hospital policies and procedures. Reports To: Director, Case Management Supervises: none

Licensure/Certification/Registration: * * Current California RN License. *Education: * Bachelor’s Degree preferred
*Experience: * * Minimum of 5 years recent patient care experience is preferred. * One-year acute hospital case management preferred.
*Special Qualifications: * * Able to demonstrate flexibility and adaptability. * Able to demonstrate excellent verbal and written communication skills. * Working knowledge of current health care issues. * Demonstrate ability to perform with minimal supervision. * General office, filing and clerical abilities to maintain accurate records.
